Office Policies
No-show policy: We charge a fee of $50 for no-show if scheduled and confirmed appointments are not cancelled or rescheduled within 48 hours.
Controlled substance prescription: We generally do not prescribe controlled substances by telehealth as this usually require in-office drug screening.
Administrative forms such as FMLA forms/other forms not directly related to visit are associated with fees depending on complexity and number of pages.
